Starting Without a Clear Business Problem

What it looks like: "We should be doing something with AI." A team is assembled. A model is built. It's technically impressive. But nobody can explain what business outcome it improves or how it connects to revenue, cost, or customer experience.
Why it happens: AI hype creates pressure to "do AI." Technology teams are excited about the technology itself. Business leaders feel they're falling behind competitors.
How to avoid it:
Start with the problem, not the technology. Before any AI work begins, answer these questions:
- What specific business problem are we solving?
- How is this problem handled today, and what does it cost?
- What does success look like in measurable terms?
- Who benefits from solving this problem?
- Would they actually use an AI solution?

Poor Data Quality and Availability
AI runs on data. Bad data produces bad AI. This seems obvious, but it trips up more projects than any technical challenge.
What it looks like: The project starts with assumptions about what data is available. Months in, the team discovers the data is incomplete, inconsistent, poorly labelled, or trapped in systems that won't share it.

Why it happens: AI possibilities feel limitless. Stakeholders pile on requirements. Each demo generates new ideas. Nobody wants to say no when the technology seems capable.
How to avoid it:
Phase ruthlessly. Define an MVP (minimum viable product) that delivers clear value in 8-12 weeks. Deploy it. Learn from it. Then expand.

- Phase 1 (8-12 weeks): Solve one specific problem well. Get it into production.
- Phase 2 (next 8-12 weeks): Expand based on what you learned in Phase 1.
- Phase 3 onwards: Scale and extend based on proven value.
Each phase should deliver measurable value. If Phase 1 doesn't work, you've invested 12 weeks, not 12 months.
Lock the scope. New ideas go into a backlog for future phases, not into the current sprint. This requires discipline from project sponsors and stakeholders.
Ignoring the Human Element
What it looks like: A technically excellent AI system is built and deployed. Users don't use it. Or they use it incorrectly. Or they actively work around it. Adoption is 15% after three months.
Why it happens: The team focused on the technology and forgot that humans need to interact with it. Change management was an afterthought - or wasn't thought about at all.
How to avoid it:
Involve end users from day one. Not just as requirements providers, but as design partners.
User adoption checklist:
- Identify all user groups and their workflows
- Understand how the AI system fits into existing work
- Design the user experience, not just the algorithm
- Pilot with willing early adopters
- Gather feedback and iterate before broad rollout
- Provide training and support

Underestimating Integration Complexity

Why it happens: AI proof-of-concepts are often built in isolation. They use clean data, run in controlled environments, and don't need to talk to other systems. Production is different.
How to avoid it:
Plan for integration from the start. Before building the AI component, map:
- What systems does the AI need to connect to?
- What APIs are available (or need to be built)?
- What data formats and protocols are used?
- What security and authentication is required?
- Who owns the systems you need to integrate with?
- What's their capacity to support integration work?
Budget integration time. In our experience, integration typically accounts for 40-60% of total project effort. If your plan has integration as a minor line item, revise it.

No Production Infrastructure
What it looks like: The AI model works on a data scientist's laptop. But there's no plan for how it runs in production - hosting, scaling, monitoring, updating, security, backup.
Why it happens: Data science teams focus on model development. Production infrastructure requires different skills - DevOps, cloud architecture, security engineering. These are often not part of the AI team.
How to avoid it:
Think about production from day one. Key questions:
- Where will the AI system run? (Cloud, on-premise, edge)
- How will it scale with load?
- How will you monitor performance?
- How will you update models?
- What happens when it goes down?
- Who operates it day-to-day?
Build the production pipeline early. We advocate deploying a simple version to production early, then improving it. This forces the team to solve infrastructure problems when they're small.
Lack of Ongoing Monitoring
What it looks like: The AI system is deployed and everyone moves on. Six months later, performance has degraded because the data distribution has changed, customer behaviour has shifted, or the world has moved on. Nobody noticed because nobody was watching.
Why it happens: Projects have end dates. Teams disband. Budget was allocated for build, not for run. Monitoring AI in production requires different tooling and skills.
How to avoid it:
Budget for operations from the start. AI systems need ongoing attention:
- Performance monitoring (is accuracy holding?)
- Data drift detection (has the input data changed?)
- Model retraining schedule
- Error analysis and correction
- User feedback collection and review
Set up automated alerts. Don't rely on someone remembering to check. Monitor key metrics and alert when they degrade.
Plan the operational model. Who is responsible for the AI system after go-live? Do they have the skills and tools they need?
Choosing the Wrong AI Approach
What it looks like: The team builds a custom deep learning model for a problem that could be solved with simple rules, or uses ChatGPT for a problem that needs a specialised model, or builds from scratch when a pre-trained model would work.
Why it happens: Teams gravitate toward what they know or what's trendy, rather than what's appropriate for the problem.
How to avoid it:
Match the approach to the problem. A rough hierarchy:
- Can rules solve it? If yes, use rules. They're explainable, maintainable, and reliable.
- Can a pre-trained model solve it? If yes, use one. Don't train from scratch when Azure OpenAI, Claude, or similar can handle it with good prompting.
- Can fine-tuning solve it? If a pre-trained model is close but not quite right, fine-tuning is often the answer.
- Do you need a custom model? Only if the above options can't work. Custom models need more data, more time, and more expertise.

Insufficient Testing
What it looks like: The AI system passes a few test cases and gets deployed. In production, it encounters scenarios the team didn't anticipate. Customer complaints follow.
Why it happens: AI testing is different from traditional software testing. You can't enumerate all possible inputs. Edge cases are harder to predict. Performance can vary across different segments of users or data.
How to avoid it:
Test comprehensively:
- Functional testing: Does it produce correct outputs for known inputs?
- Edge case testing: What happens with unusual, malformed, or adversarial inputs?
- Bias testing: Does it perform equally across different demographic groups?
- Load testing: Does it perform under production-level demand?
- Integration testing: Does it work correctly within the broader system?
- User acceptance testing: Do actual users find it useful and usable?
Test with production-like data. Synthetic or sanitised test data often misses the messiness of real-world inputs.
No Executive Sponsorship
What it looks like: The AI project is championed by a mid-level manager or a technology team. When it needs cross-departmental cooperation, budget increases, or organisational change, there's nobody with authority to make it happen.
Why it happens: AI projects often start as experiments or technology initiatives. They don't get senior sponsorship until they've already stalled.
How to avoid it:
Get executive sponsorship before you start. The sponsor should:
- Own the business outcome the AI project is targeting
- Have authority over budget and resources
- Be able to drive cross-departmental cooperation
- Be willing to champion the project when things get difficult
- Understand that AI projects involve experimentation and iteration
Without this, the project is fragile. One organisational change, one budget review, one political disagreement, and it stalls.
